Orthodontic Registration Form
We look forward to meeting you on your first visit to our office. During this time, you will get acquainted with our office and learn about your possible orthodontic treatment. We understand that each patient's needs are different, and we promise to give you the personal attention you deserve. Your first orthodontic visit will consist of an examination and discussion of potential treatment options. This important visit will address your orthodontic needs to determine if this is the proper time to begin treatment.
If treatment is recommended we will gather the necessary diagnostic records. These records consist of study models of your teeth ("molds"), photographs and x-rays. After we evaluate your records, we will explain all aspects of your treatment in detail. We will provide a complete clinical diagnosis regarding the nature of the problem, a written summary of the findings and a detailed plan that outlines your treatment, goals and estimated fees. We encourage questions from both the patient and the parent, and as such, we provide ample time to answer your questions.
Digital X-Rays
At Commerce Park Children's Dentistry & Orthodontics, we use one of the latest technological advances in dentistry - digital radiography (X-rays.) Digital X-rays are much safer for the child, producing significantly lowering radiation exposure compared to conventional x-rays. Digital technology also allows us to enhance or enlarge the X-ray for better viewing, as well as to share the X-ray with referring dentists via e-mail.
